The story begins in 1974 at the Glenville Sanatorium, a remote mental institution that houses patients with severe deformities and violent tendencies. Among them are the notorious hillbilly cannibals: Three-Finger, One-Eye, and Saw-Tooth. The staff struggles to control the inbred mutants, but one night, the patients revolt, killing the doctors and nurses in a gruesome massacre. The cannibals take over the facility, using it as their home.

Years later, in 2003, a group of college friends—Kenia, Sara, Jenna, Vincent, Kyle, Claire, Daniel, Bridget, and Lauren—decide to take a snowmobiling trip during winter break. A snowstorm forces them to seek shelter in what they believe to be an abandoned building, which turns out to be the same sanatorium from the opening events. Unaware of the building’s dark past, they explore the eerie, decaying halls, only to realize too late that they are not alone.The cannibal brothers, still residing in the sanatorium, begin stalking and attacking the group one by one. The friends attempt to fight back, but their unfamiliarity with the building and the relentless brutality of the killers put them at a severe disadvantage. As tensions rise and their numbers dwindle, the survivors try to escape the snowbound facility before they become the next victims.
